Vitamin D deficiency may be one factor in this increased frequency of infections in children. The high prevalence and lack of outward manifestations of vitamin D deficiency may explain the low recognition of this condition.</p> <p><bold>Aim.</bold> Assessment of the relationship of vitamin D status with the incidence of RI in infants and young children.</p> <p><bold>Materials and methods.</bold> The study included 94 children aged 4.5–18 months (mean age – 10.9±2.66 months). We determined vitamin D levels in infants and young children and the number of RI episodes in the first six months of life. The children belonged to health groups I and II. The association between the incidence of RI and vitamin D levels was analyzed: the average values of vitamin D levels in children in subgroups with different frequencies of RI were determined, and a critically significant vitamin D level associated with the development of 4 or more episodes of respiratory tract infections during the follow-up period was revealed.</p> <p><bold>Results.</bold> The average blood vitamin D level in children was 48.33±21.16 ng/ml. Vitamin D deficiency was detected in 5 (5%) children, and insufficiency was found in 14 (15%). Children with one disease in the first six months of life had the highest average vitamin D levels – 57.1 (49.4–69.7; CI 95%) ng/ml, which was statistically significant in comparison with the group of children with three or more diseases – 38,1 (36,9–60,1; CI 95%) ng/ml. The vitamin D level was determined to be 37 ng/ml, a decrease in which is associated with a statistically significant increase in the incidence of RI – more than 4 episodes in 6 months.</p> <p><bold>Conclusion.</bold> Vitamin D deficiency and insufficiency in children are associated with more frequent episodes of RI. Vitamin D levels below 37 ng/ml are associated with frequent episodes of acute RI in the first six months of life (more than five episodes per six months). Thus, the frequency of RI may be a marker of vitamin D deficiency or insufficiency in the absence of clinical symptoms affecting the musculoskeletal system.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Заболеваемость респираторными инфекциями (РИ) среди детского населения имеет стабильно высокие значения в течение многих лет. Одним из факторов повышения частоты инфекций у детей может являться недостаточность витамина D в организме. В то же время высокая распространенность и отсутствие явных внешних проявлений недостаточности витамина D могут вносить вклад в гиподиагностику данного состояния.</p> <p><bold>Цель.</bold> Оценить взаимосвязь статуса витамина D с заболеваемостью РИ у детей грудного и раннего возраста.</p> <p><bold>Материалы и методы.</bold> В исследование включены 94 ребенка в возрасте 4,5–18 мес (средний возраст – 10,9±2,66 мес). В ходе исследования определен уровень витамина D в сыворотке крови методом высокоэффективной жидкостной хроматографии у детей грудного и раннего возраста, принадлежащих к 1 или 2-й группе здоровья. Фиксировалось количество эпизодов РИ в первые 6 мес жизни. Проанализирована ассоциация между заболеваемостью РИ и уровнем витамина D: определены средние значения уровня витамина D у детей в подгруппах с разной частотой РИ, выявлен критически значимый уровень витамина D, ассоциированный с развитием 4 и более эпизодов инфекций респираторного тракта за период наблюдения.</p> <p><bold>Результаты.</bold> Среднее значение уровня витамина D у обследованных детей в крови составило 48,33±21,16 нг/мл. Дефицит витамина D выявлен у 5 (5%) детей, недостаточность – у 14 (15%). В группе детей с 1 заболеванием за первое полугодие жизни определены статистически значимо более высокие значения уровня витамина D – 57,1 (95% доверительный интервал 49,4–69,7) нг/мл в сравнении с группой детей с 3 заболеваниями и более, в которой средние значения составили 38,1 (95% доверительный интервал 36,9–60,1) нг/мл (<italic>p</italic>&lt;0,05). Определен уровень витамина D – 37 нг/мл, ниже которого у обследованных детей отмечено статистически значимое увеличение заболеваемости РИ – более 4 эпизодов за 6 мес жизни.</p> <p><bold>Заключение.</bold> Недостаточность и/или дефицит витамина D в организме ребенка ассоциированы с более частыми эпизодами РИ. Уровень витамина D&lt;37 нг/мл сопряжен с частыми эпизодами РИ у детей грудного возраста (более 5 за 6 мес).
