OME is a disease of the middle ear, which is characterized by the accumulation of exudate behind the tympanic membrane in the absence of perforation for three months or longer, leading to hearing loss, increasing the risk of delayed formation of correct speech in the child and behavioral problems. The question of the development of OME, in view of the presence of an infectious focus in the adenoid and further spread to the auditory tube, is ambiguous. In some works, the possible spread of the inflammatory process to the Eustachian tube with long-term preservation of pathogenic microorganisms in the structure of the lymphoid tissue, which is characteristic of persistent viral infections, and the further development of OME are considered. The mucous membrane of the nasopharynx is inhabited by many microorganisms: representatives of an indigenous microbiota, opportunistic and pathogenic.</p> <p><bold>Aim. </bold>To study the microbiome of the mucous membrane of the nasopharynx and middle ear in children with chronic adenoiditis (CA) and OME.</p> <p><bold>Materials and methods. </bold>The study included 174 children with OME and CA (114 boys and 60 girls) aged 3 to 14 years (6.9±0.5 years). We have developed a staged diagnostic algorithm, including the collection of a life history, illness and complaints of the child (or his parents), as well as ENT examination, endoscopy of the nasopharynx, otoendoscopy, audiological examination, microbiological examination by real time PCR, whole genome sequencing NGS.</p> <p><bold>Results. </bold>The main microorganisms of the nasopharyngeal mucosa in children with CA and OME of different age groups are described.</p> <p><bold>Conclusion. </bold>In the treatment of children with CA and OME, it is necessary to take into account the features of the normal microbiota of the nasopharynx; it is important to understand that by acting on opportunistic and pathogenic microorganisms, it is necessary to remember to create favorable conditions for stimulating the growth and development of representatives of the indigenic microbiota, which in turn will contribute to the patient’s speedy recovery and absence of relapses in the future.</p></abstract><trans-abstract xml:lang="ru"><p>Роль патологии глоточной миндалины в развитии экссудативного среднего отита (ЭСО) в настоящее время обсуждается. ЭСО – это заболевание среднего уха, для которого характерно скопление экссудата за барабанной перепонкой при отсутствии перфорации в течение 3 мес или дольше, приводящее к снижению слуха, что увеличивает риск задержки формирования правильной речи у ребенка и поведенческих проблем. Вопрос о развитии ЭСО ввиду наличия инфекционного очага в глоточной миндалине и дальнейшего распространения на слуховую трубу неоднозначен. В некоторых работах рассматриваются возможное распространение воспалительного процесса на евстахиеву трубу при длительном сохранении в структуре лимфоидной ткани патогенных микроорганизмов, что характерно для персистирующих вирусных инфекций, и дальнейшее развитие ЭСО. Слизистую оболочку носоглотки заселяет множество микроорганизмов: представителей индигенной микробиоты, условно-патогенной и патогенной.</p> <p><bold>Цель. </bold>Изучить структуру микробиома слизистой оболочки носоглотки у детей с хроническим аденоидитом (ХА) и ЭСО.</p> <p><bold>Материалы и методы. </bold>В исследование включены 174 ребенка с ХА и ЭСО (114 мальчиков и 60 девочек) в возрасте от 3 до 14 лет (6,9±0,5 года). Всем детям проведен сбор анамнеза заболевания, жалоб ребенка (или его родителей), стандартный ЛОР-осмотр, включающий эндоскопию полости носа и носоглотки, тимпанометрию и полногеномное секвенирование NGS мазков из носоглотки. Оценку клинической эффективности проводили согласно полученным объективным данным и данным визуально-аналоговой шкалы по 5-балльной системе, которую заполняли дети и их родители, где 0 баллов – отсутствие симптома, 1 балл – слабо выраженный симптом или непостоянный, 2 балла – умеренно выраженный симптом, 3 балла – выраженный симптом, 4 балла – сильно выраженный симптом или постоянный, и микробиологических данных: на 10-е сутки, через 1 мес, через 3 мес.</p> <p><bold>Результаты. </bold>Описаны основные микроорганизмы слизистой оболочки носоглотки у детей с ХА и ЭСО разных возрастных групп, а также изменения структуры микробиоты в зависимости от вида консервативной терапии.</p> <p><bold>Заключение. </bold>При лечении детей с ХА и ЭСО необходимо учитывать особенности нормальной микробиоты слизистой оболочки носоглотки; важно понимать, что, воздействуя на условно-патогенные и патогенные микроорганизмы, необходимо помнить о создании благоприятных условий для стимуляции роста и развития представителей индигенной микробиоты, что, в свою очередь, будет способствовать скорейшему выздоровлению пациента и отсутствию рецидивов в дальнейшем.</p></trans-abstract><kwd-group xml:lang="en"><kwd>otitis media effusion</kwd><kwd>chronic adenoiditis</kwd><kwd>microbiome</kwd><kwd>microbiota</kwd><kwd>genome sequencing</kwd><kwd>microorganisms</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>экссудативный средний отит</kwd><kwd>хронический аденоидит</kwd><kwd>микробиом</kwd><kwd>микробиота</kwd><kwd>полногеномное секвенирование</kwd><kwd>микроорганизмы</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Ивашкин В.Т., Ивашкин К.В.
